Helen Mathews is found strangled on the street. In the course of the investigation, the police learn that she was having an affair with Alan White, a flying instructor working for her husband Michael. From the evidence, it appears that she was about to leave her husband and move to Canada. The police uncover a similar case from several years earlier with a striking resemblance to this case and are convinced that the two must be connected. Friction again develops between Sam Ryan and the police when her forensic analysis demonstrates that the two cases have no connection whatsoever. Sam continues to fret over her decisions in her last case and her personal relationship with Peter Ross reaches a breaking point.

With a second murder on their hands, the police make an arrest in the case. Sam however uncovers some very peculiar bruising on both victims that provides the police with the evidence they need to identify the person responsible. Sam's life is put at risk however when the murderer threatens her. Wyn Ryan, who has been living with Sam since their mother's death, has found a job and is now looking to move on. Sam and Peter continue to struggle with their relationship but are determined to make a go of it.

This is a recap and follow-up to an earlier episode (Dateline 25.20) dealing with the case of Michelle Hadley who was arrested for sending her ex-husband Ian Diaz and his new wife Angela threatening emails. The case took a twist when the police found evidence that pointed to someone else as the culprit. In this latest episode, it's revealed that the culprit pleaded guilty and was sentenced to 5 years in prison.

When 72-year old Millicent Lockwood is found brutally beaten to death in a local park, the police immediately focus on a local who was seen arguing with her a short time earlier. The autopsy reveals little evidence but Sam Ryan once again refuses to make the evidence they do have fit the suspect, which leads to an inevitable clash with the police. She also sees two youths, Kelvin Price and Ben Quayle, act out part of the murder. The police however are pursuing other lines of inquiry and don't have the time, nor the inclination, to pursue Sam's theories. She then sets off on her own investigation.

The police are concerned that her involvement may actually taint some of the evidence they have and an angry DCI Peter Ross informs her that he has filed a formal complaint with the Coroner and the Chief Constable over her conduct. This obviously affects their professional and personal relationship.

Lenny (Matt Craven) and Leslie (Aunjanue Ellis) are flagged down by a deaf woman who tells them her brother is being held at gunpoint. They follow the woman to her home where they find a hostage situation. Leslie comes close to shooting the woman's brother who has managed to get hold of the gun. Richie (Julio Oscar Mechoso) and Gayle (Catherine Kellner) are at the scene of a major auto accident where a small boy and his father are trapped in their van and a utility pole has been knocked down. Gas is everywhere and the power line is sparking. Finally power to the area is turned off and Richie is able to free the boy and his father. Marsh (David Keith) and Willitz (Cole Hauser) stumble upon an armed robbery and chase the robber through a church parking lot and into a public library where Russell (Louis Mustillo) and Terry (Matthew Beck) are in the vicinity. Lenny, who is covering the back exit, comes face to face with the robber and is overcome with memories of his partner's death, letting the criminal get away. After the incident he decides to get psychiatric help.

After Grizz tells the wife that Mike's a former cop, she flees to New Jersey where Mike quickly tracks her down. Upon their return to Philly, Marcellus arrests them for kidnapping but is forced to release them when neither one will reveal the whereabouts of the little girl. Marcellus connects Mike with the cop, who tells a story that resonates uncomfortably with Mike's recollection of his own behavior, which is confirmed by his son. Troubled, he talks to Grizz and then follows his advice to make amends with Heather. When Mike sets a trap to discover the truth about the cop's abuse, he finds that the truth is not always what it seems.
