Mitch Buchannon didn't die in a boat explosion in the 10th season but has been recovering from amnesia in Los Angeles. Mitch returns to Hawaii to get married again! His new fiancée Allison looks a lot like Lt. Stephanie Holden, a past love. Little does he know, Allison is cooperating with Mason Sato who appeared in the show's 2nd season and now returns for revenge. The Baywatchers are invited to Hawaii by CJ. She's opening CJ's Bar & Grill at the Turtle Bay Resort. Coincidentally, Caroline Holden is already there filming a soap opera. Neely Capshaw (ex-Mrs. Buchannon) comes to Hawaii to stop the wedding. She wants Mitch back!

Former County General alumna Dr. Susan Lewis returns to Chicago looking for a new job and swings by her old digs to renew friendships -- especially with Dr. Greene -- while a nervous Dr. Chen is accompanied by Dr. Weaver to a "risk management" review of her treatment of a deceased patient. Elsewhere, Dr. Benton is caught between a life-saving surgery and the need to pick up his young son before his school locks up. Dr. Corday meets Dr. Lewis and struggles with the parents of a young boy with a rare and chronic disease who have grown discouraged with his long-term prognosis. Dr. Kovac is surprised when a beautiful French waitress shows up in the ER and asks for him.

Val refuses to leave, so Ben waits with her. He tells Val that Karen believed her all along, and worked for months to find the babies. Val's upset that no one told her they believed her. Harry arrives home with a police escort, who tell Val she has to leave. She loses it. At home, Joshua tells Val that her babies were found because he and his viewers prayed that she would be forgiven of her sins. Val drives back to the Fishers and waits all night in her car. Mac has Karen bring Val to the hospital to get the birth records, but the clerk says there are no records of the twins.
